Eternal Diagnostics Lab, LLC is responsible for the drawing and delivery of laboratory specimens. Eternal Diagnostics Lab, LLC does not collect any specimens without a physician order or a request from a laboratory. Eternal Diagnostics Lab, LLC is not allowed to give any medical advice, treat, or diagnose anyone.

Eternal Diagnostics Lab, LLC needs a copy of the order 24 hours before scheduled appointment,and patient informatin,copy of the insurance card (front and back) and paitent ID at the scheduled appointment. If we do not receive the requested information in the time range mentioned, then the scheduled appointment may havd to be reschedule. This is extremely important so that we have adequate time to prepare for the blood draw and that we perform our job correctly.

Eternal Diagnostics Lab, LLC handles the delivery of all samples to their proper laboratory. If a sample is being collected for a laboratory that needs to be picked up via courier service, Eternal Diagnostics Lab, LLC is not responsible for any delay of arrival to the laboratory, due to weather conditions or any other problems that may occur once the sample has been delivered or picked up by courier service.

Eternal Diagnostics Lab, LLC is not responsible for any errors that may occur at the laboratory. It is the processing labs’ responsibility to process the specimen once received by Eternal Diagnostics Lab, LLC. In the event of a processing lab error, Eternal Diagnostics Lab, LLC would gladly return tbe happy to recollect the sample. Any error made by Eternal Diagnostics Lab, LLC  (drawing the wrong tubes) will be corrected by re-drawing at no extra charge; as long as all correct paperwork was submitted adequately before appointment.

Any billing questions or problems for tests collected for this service should be directed to your insurance company or the laboratory billing department. Eternal Diagnostics Lab, LLC is a private pay service. Specimen collection services are provided and billed by Eternal Diagnostics Lab, LLC; Testing is billed through your insurance. 

Please be advised that Eternal Diagnostics Lab, LLC is not responsible for any outstanding balances with LabCorp or Quest and this will need to be settled prior to a scheduled lab draw. The processing labs have the right to decline specimens of patients with outstanding balances.

Eternal Diagnostics Lab, LLC is an independent convenience service. We are fully HIPPA compliant and respect the privacy of our clients. Under no circumstances Eternal Diagnostics Lab, LLC will give out client’s personal information to unauthorized personnel. All client information is always kept personal and confidential.

Eternal Diagnostics Lab, LLC does not receive any results. In the event that your doctor did not receive results, it is the responsibility of the Doctor’s office to get in touch with the lab. Most of the time they have an account with the labs. 

Eternal Diagnostics Lab, LLC can change terms and conditions at any time.
